What do I need to do when my policy matures?

Sony Life will notify you through mail on when you can get the proceeds of your policy. In claiming your check, present the letter and show any valid ID.

How to file death claim?

The following are the basic requirements:

 

  • Insured’s death certificate duly sealed by the Office of Local Civil Registrar
  • Policy Contract
  • Birth or Baptismal Certificate
  • Claim Forms

 

  • Claimant Statement
  • Certificate of Attending Physician
  • Certificate of Identifying Witness
