What if I change my address and contact nos.?

Notify Sony Life right away by either of the following:

 

A.) Fill up the back portion of the billing / premium notice or make a letter of request, then fax this at 819-1805, mail at our office address c/o Policy Services Department, or send through your Lifeplanner; or,

 

B.) Simply inform your Lifeplanner.

 

You should receive a letter to confirm your new address and contact nos.

How can I correct my personal information?

Accomplish the Name Change Request Form which you can obtain from the Sony Life Home Office or through your Lifeplanner. Submit a copy of legal documents such as birth, baptismal, or marriage certificate or an affidavit correcting the facts.

How can I change the mode of premium payment?

Accomplish Request for Change 1 Form and pay the chosen modal premium. The new mode should conform to the cycle of the original mode of the policy. Please inquire from your Lifeplanner or from our Policy Services Department for more details.

What will I do if I want to request for a change in plan, face amount or any amendment in my policy?

Accomplish Request for Change 2 Form. This should be signed and dated by all irrevocable beneficiaries and assignees, if any. Submit the Policy Contract for us to effect the necessary changes. You may be required to submit Statement of Insurability or other proof of good health. If the policy change will result to additional benefit, Sony Life may ask for additional requirements.

What do I need to do if my policy contract is lost?

Accomplish Declaration of Loss of Policy Contract Form and let all irrevocable beneficiaries and assignees, if any, sign the form. Pay the policy contract fee as you apply for a duplicate copy of your contract.

How will I change my beneficiaries?

Accomplish Change of Beneficiary Form indicating new beneficiaries, their age, relationship to the insured, and % of share. Have the form duly signed, dated, and witnessed by the policy owner and all irrevocable beneficiaries and assignees, if any.


If the beneficiary is a minor (below 18 years old), a trustee has to be designated by indicating his/her name and relationship to beneficiary. Submit the Policy Contract to facilitate processing.


NOTE: A trustee is only a representative of the minor beneficiary in documenting claims. He/she has no authority over the minor while insured is still living. Immediate relatives can be designated trustee for a minor.

Who are the persons that I can designate as my beneficiaries?

Only those who have insurable interest on the life of the insured, whereby death of the insured would result to a genuine economic loss on their part, can be designated as beneficiaries. Common-law spouse, a person found guilty of adultery or concubinage, or as accomplice in a crime cannot be named as beneficiary.

How can I assign my policy to a creditor?

Accomplish Assignment for Collateral Form. Have it notarized and countersigned by all the irrevocable beneficiaries and assignees, if any.

 

Submit the form, loan agreement, corporate secretary’s certificate of the creditor’s authorized signatories, and Policy Contract.

How can I request for the cancellation of the assignment of my policy?

Submit the Policy Contract with an affidavit executed by the assignee relinquishing interest on the policy.
